Repeat Prescriptions (Please give 48 hours notice)
To avoid errors and confusion please avoid using the telephone to make your request. It is safer for you if you either make your request by letter or personally calling at reception. We are now fully computerised and whenever you receive a repeat prescription you will also receive a PRINT OUT LEAFLET which details all of your medication.
If you want your prescription to be sent to you in the post, please enclose a stamped addressed envelope.
PLEASE AVOID REQUESTING AND COLLECTING REPEAT PRESCRIPTIONS DURING CONSULTING HOURS AS THE SURGERY AND DISPENSARY ARE VERY BUSY AT THIS TIME.
Certificates, Claim Forms Etc
If you require medica certificates, claim forms, insurance certificates etc., to be completed by the Doctor please discuss this with the receptionist who will advise you if you need to see the Doctor, or if a fee is payable.
